Beyond Collaborative Care connects you with personalised allied health services for NDIS participants.

Every participant has different goals, circumstances and support needs. When we receive your referral, our team will review the information provided to determine whether we have the right service, clinical expertise and availability to support you safely and effectively.

You may be eligible for our services if:

• You have a current NDIS plan.

• Your plan contains suitable funding for the requested service.

• You live within one of our service regions.

• The support you require is within our team’s professional scope.

• We have an appropriate clinician available to meet your needs.

• The requested service can be delivered safely and in a way that supports your goals.
   

Eligibility and service availability are confirmed after our team reviews your referral.
